It's because you've Radeon Graphics. Disable it

1) my laptop freeze when it goes to Sleep/wake & lid close/open
You've Radeon Graphics, that is making an issue. After you follow the above guide, but still your graphics will be active while you put your system into sleep and wake. So the thing you need to do is enable Radeon graphics on sleep and again disable it on wake. At the end of this guide, you'll see the fixing sleep and wake issue.

3) even though the sound is OK when I use the speakers, when is use headphones there is no sound.
How you're enabling your sound in your Hackintosh? AppleALC.kext or VoodHDA.kext? If you're using AppleALC.kext which layout-id you're using?

4) I have managed to use the integrated GPU an intel but my system has also a Radeon 530
Radeon 530 is disabled, it is not supported. If you've got the option to disable it via BIOS then there's no need of DSDT patches as I linked links above, all your problems will be solved.
